This position provides care to patients in:

Elkhart County
LaGrange County
Surrounding communities

Travel is within the local service area.

What You Will Do

Provide skilled nursing care in patients homes under RN supervision
Follow physician orders and plan of care
Educate patients and caregivers on treatment and self-care
Monitor patient condition and report changes to the RN Case Manager
Complete accurate and timely clinical documentation
Promote patient safety and help prevent hospital readmissions
Build trusting relationships with patients and families

What You Bring

Active Licensed Practical Nurse (LPN) license
Graduate of an accredited nursing program
1 year of clinical experience preferred
Home Health or Hospice experience preferred
Ability to work independently in the field
Valid drivers license and acceptable driving record
Ability to lift and assist patients as needed
