What You Will Do

Manage and coordinate your patient caseload
Complete start of care visits and follow-up assessments
Develop and update individualized care plans
Monitor patient condition and communicate with physicians
Educate patients and families on treatment plans
Maintain accurate clinical documentation
Support patients throughout their care journey

What You Need

Current Registered Nurse RN license
1 year of clinical nursing experience
Ability to travel within the branch service area
Valid drivers license and acceptable driving record
Ability to lift push pull or support up to 50 pounds
Associate or Bachelors degree in Nursing preferred
Home Health or Hospice experience preferred
HomeCare HomeBase experience preferred
