SUMMARY

An Interim Case Coordinator will help people meet their personal needs and goals by obtaining the most significant degree of independence and inclusion possible in everyday life. They assist individuals with developmental disabilities and their families in identifying services and support to meet their needs. They advocate monitor and evaluate services provided to the individual.

  • Provides coverage due to a staff vacancy or extended leave situation by explicitly supporting caseloads in need of temporary coverage.
  • Temporarily manages cases of varying ages disabilities and case complexity to improve the quality of support and services by quickly developing rapport and trust through contact and planning with the individual and their team to identify needs and create an action plan to assist the person in meeting those needs.
  • Has established and/or develops community relationships and support systems and if appropriate links resources by matching the unique support needs of individuals and families (identified in the plan) with resources in the community. Linking may involve researching existing resources developing new resources making referrals to collaborating agencies with information and follow-up support preparing Utilization Review requests and service authorizations using comprehensive knowledge of available funding sources and coordinating federal state county and community programs to achieve necessary support.
  • Ensures quality enhancement by monitoring and assessing the quality timeliness and effectiveness of services and support received through an ongoing review process. Monitoring occurs via phone or email contact as well as through in-person -person meetings are completed Monthly Quarterly and annually. These meetings may occur in a variety of settings including but not limited to individuals homes day programs community settings office settings group homes and/or residential settings. If there are findings from the monitoring reporting processes will be followed including protocols for mandated reporting in the case of suspected abuse or neglect.
  • Maintain accurate and timely documentation by recording all activities delivered within three business days of TCM activity and maintaining a minimum of 121 TCM billable hours per month. Due to the nature of this position most hours worked would qualify as billable time.
  • Provides the most up-to-date information to people we support by participating in knowledge enrichment such as completing all required training.
  • Creates team unity by accepting and willingly carrying out special assignments or duties.

QUALIFICATIONS

REQUIRED EDUCATION

  • A Bachelors degree from an accredited college or university or Registered Nurse license.

REQUIRED MISCELLANEOUS QUALIFICATIONS

  • The selected applicant must have reliable transportation and an acceptable driving record. Must be able to drive for work-related assignments to individuals homes to meet face-to-face with individuals on caseloads.
  • Must have a valid drivers license. Must provide proof of personal valid vehicle insurance.
  • Must have access to reliable internet for telecommuting employees.

REQUIRED EXPERIENCE

  • Must have at least one year of experience in working with individuals with intellectual and/or developmental disabilities in a community-based setting as a Registered Nurse or in social work special education psychology counseling vocational rehabilitation physical therapy occupational therapy speech therapy or a closely related area.
  • Experience with a variety of case management and documentation applications such as but not limited to Setworks Kronos Therap etc.
  • Experience as a member of a multi-disciplinary team in the areas of assessing developing or implementing individual person-centered plans especially for individuals with intellectual/developmental disabilities.
  • Experience in utilizing the Microsoft Office Suite
