Position Title: Licensed Practical Nurse (LPN) Long-Term Care
Location: Vestal New York
Job Type: Contract (Local or Travel)
Duration: 16 Weeks
Shift: Evening Shift 2:30 PM 11:45 PM
Position Overview
Navitas Healthcare LLC is seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) for a 16-week contract assignment in a Long-Term Care / Skilled Nursing Facility setting. The LPN will provide direct patient care under the supervision of a Registered Nurse (RN) or physician ensuring high-quality care and patient safety in a fast-paced environment.
Key Responsibilities
- Perform focused patient assessments and report any changes in condition to the RN or healthcare provider
- Administer medications treatments wound care and IV therapies in accordance with scope of practice and facility guidelines
- Assist in the development and implementation of individualized patient care plans
- Collaborate with RNs physicians and interdisciplinary teams to support patient care and discharge planning
- Accurately document patient care activities observations and responses in the EMR system
- Provide basic patient and family education as directed by the RN
- Respond promptly to patient needs and communicate effectively with the healthcare team
- Maintain compliance with infection control protocols medication administration standards and safety regulations
- Assist with patient admissions transfers and discharges
- Support unit operations and contribute to a positive team environment
Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of New York
- Basic Life Support (BLS) certification required (AHA preferred)
- Minimum 1 year of recent LPN experience in a hospital or long-term care setting preferred
- Strong knowledge of basic nursing practices and clinical procedures
- Ability to work effectively under supervision in a team-based environment
- Proficiency in EMR systems and standard documentation practices
- Excellent communication time-management and organizational skills
