Job Title: Registered Nurse (RN) Med/Surg Homecare

Schedule: Day Shift MondayFriday 9:00 AM 5:00 PM
Hours: 40 hours/week

Job Summary:

We are seeking an experienced Registered Nurse (RN) to provide high-quality patient-centered care in a homecare setting. The RN will deliver direct nursing care to pre- and post-surgical patients in their homes ensuring safe recovery effective treatment and patient education.

Responsibilities:

• Provide individualized bedside nursing care in patients homes
• Assess plan implement and evaluate patient care plans
• Administer medications and monitor for adverse reactions
• Perform wound care dressing changes and infection control procedures
• Initiate and manage IV therapy and catheter care
• Monitor and document patient vital signs and overall condition
• Adjust and manage medical equipment as needed
• Educate patients and families on post-operative care and recovery
• Collaborate with healthcare teams for care coordination and discharge planning
• Maintain accurate and timely documentation
• Manage a daily caseload of approximately 48 patients

Requirements:

• Active New York State RN License with Primary Source Verification
• Current BLS certification (AHA) required
• Minimum 2 years of recent Med/Surg experience in an acute care or post-acute setting
• Must have true homecare experience (providing care in patient homes)
• Strong skills in physical assessment IV therapy wound care and patient education
• Experience with telemetry monitoring preferred
• Excellent communication and organizational skills
