Job Title: LPN / LVN Long-Term Care
Location: Vestal NY 13850 USA
Duration: 16 Weeks
Schedule: 5x8-hour shifts Every Other Weekend
Shift: Day Shift (06:30 23:45)
Job Summary
Navitas Healthcare LLC is seeking a dedicated LPN/LVN to provide high-quality nursing care in a long-term care and rehabilitation setting. The role includes medication administration patient monitoring and supporting residents with complex needs including Alzheimers and dementia care. Ideal candidates are detail-oriented compassionate and thrive in a fast-paced team-driven environment.
Key Responsibilities
Patient Care & Monitoring
- Provide direct nursing care to residents
- Monitor patient conditions and report changes
- Assist with activities of daily living (ADLs)
- Deliver compassionate care especially for dementia and Alzheimers patients
Medication Administration
- Administer medications accurately and safely
- Document medication administration per protocol
- Monitor for effectiveness and adverse reactions
Clinical Support
- Assist with care planning and implementation
- Collaborate with interdisciplinary teams
- Float between units as needed
- Maintain accurate EMR documentation
Facility Operations
- Follow infection control and safety standards
- Comply with facility policies and procedures
- Participate in training and orientation programs
Communication & Teamwork
- Communicate effectively with residents families and staff
- Escalate concerns appropriately
- Support a collaborative care environment
Required Qualifications
- Minimum 1 year of nursing experience
- Experience with medication administration (Med Pass)
- Strong communication and interpersonal skills
- Willingness to work with dementia/Alzheimers patients
- Open to LTC rehab med-surg or hospital background
- First-time travelers welcome
Preferred Qualifications
- Prior long-term care experience
- Experience with PointClickCare (PCC) EMR
- Experience in large facilities (300 beds)
Education
- Diploma in Practical Nursing (Required)
Certifications & Licensure
- Active New York LPN/LVN License (Required)
- BLS Certification (Required)
- COVID-19 Vaccination (Required)
