Job Summary
Navitas Healthcare LLC is seeking a dedicated LPN/LVN to provide high-quality nursing care in a long-term care and rehabilitation setting. The role includes medication administration patient monitoring and supporting residents with complex needs including Alzheimers and dementia care. Ideal candidates are detail-oriented compassionate and thrive in a fast-paced team-driven environment.
Key Responsibilities
Patient Care & Monitoring
Provide direct nursing care to residents
Monitor patient conditions and report changes
Assist with activities of daily living (ADLs)
Deliver compassionate care especially for dementia and Alzheimers patients
Medication Administration
Administer medications accurately and safely
Document medication administration per protocol
Monitor for effectiveness and adverse reactions
Clinical Support
Assist with care planning and implementation
Collaborate with interdisciplinary teams
Float between units as needed
Maintain accurate EMR documentation
Facility Operations
Follow infection control and safety standards
Comply with facility policies and procedures
Participate in training and orientation programs
Communication & Teamwork
Communicate effectively with residents families and staff
Escalate concerns appropriately
Support a collaborative care environment
Required Qualifications
Minimum 1 year of nursing experience
Experience with medication administration (Med Pass)
Strong communication and interpersonal skills
Willingness to work with dementia/Alzheimers patients
