Speech Language Pathologist SLP Home Health PRN

Empath Home Health a member of Empath Health is seeking a compassionate PRN Home Health SLP (Speech Language Pathologist) to work with our Home Health team to deliver individualized care to patients in their homes inBrandon Riverview and surrounding areas.

Locations: Home health visits available in Brandon Riverview and surrounding areas. It is very flexible - you choose when & where you want to work!

Schedule: PRN role; 5-10 home health visits per week

What youll Do

• Perform initial and ongoing assessments including OASIS assessments at appropriate time points to identify the clients level of functioning swallowing and communication ability.
• Develop a plan of care in consultation with the physician and other care team members.
• Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
• Instruct patients and families on using special educational strategies and techniques to improve sensory-motor development perception memory language and cognition.
• Prepares clinical and progress notes of services provided patient activities and condition and submit in a timely manner.
• Collaborate with the interdisciplinary team to ensure seamless holistic care

What Youll Need

• Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
• Active Florida SLP license
• Home health care experience preferred
• Experience using Homecare Homebase (HCHB) EMR is a plus
