Position Summary
The Clinical Coordinator is responsible for supporting the delivery of high-quality patient care within the Ambulatory Clinic and through the Medly program. This role involves monitoring and managing patient alerts providing education on heart failure management coordinating care across the continuum and collaborating with interdisciplinary teams to ensure safe effective and patient-centered care. The Clinical Coordinator applies clinical expertise best practice guidelines and continuous quality improvement principles to optimize patient outcomes and support program and organizational goals.
Duties
- Monitor and manage daily Medly alerts in collaboration with the Heart Function Nurse Practitioner and Physicians including prioritization and escalation as appropriate
- Communicate with patients and families to support proper use of Medly and provide education on heart failure management and self-care strategies
- Support Guideline-Directed Medical Therapy including medication titration using Medly and clinical expertise
- Coordinate patient care across the continuum ensuring continuity and effective collaboration with interdisciplinary teams and external partners
- Perform clinical duties including vital signs monitoring IV insertion and management and administration of clinic-approved IV medications
- Recognize and respond to potentially harmful arrhythmias facilitating timely medical intervention and interpretation
- Obtain review and triage medical records to support clinical decision-making and patient flow
- Document all clinical activities and communications in accordance with College of Nurses of Ontario standards using the EPIC system
- Apply best practice guidelines and contribute to quality improvement research and academic initiatives
- Contribute to a positive work environment and support departmental goals policies and cross-functional responsibilities as assigned
Qualifications :
- Current registration with the College of Nurses of Ontario
- Bachelor of Science in Nursing; Masters preferred
- Current BCLS Certification
- Minimum Three (3) to Five (5) years Cardiology/C.V. Surgery Nursing and/or CICU/CVICU
- Strong organizational and time management skills
- Excellent interpersonal and customer service skills
- Excellent computer skills including Microsoft Office and EPIC applications
- Excellent verbal and written communication skills
- Knowledge and experience with heart failure guidelines and remote titration of HF patients an asset.
- Demonstrated ability to cultivate and build relationships
- Strong track record of high-quality work and engagement is essential
- Able to adapt to a constantly changing environment that requires ongoing revaluation of priorities
- Demonstrated ability to use good judgment in assessing difficult situations
- Able to work collaboratively within a group setting
- Proven track record of taking initiative
- Interest and commitment to health equity and to working in partnership with Indigenous peoples and communities to enable access to high-quality care.
- Completion of Cultural Safety and Principles of OCAP training are a definite asset. If not completed prior to hiring completion will be a requirement for the successful candidate.
