Deliver personalized home-based primary care from a Brooklyn hub focusing on homebound patients throughout NYC.

Position Overview

A large multispecialty group in NYC is seeking aNurse Practitioner (NP)to providehigh-quality patient-centered carein patients homes and nursing facilities. This role is ideal for providers who enjoyholistic care chronic disease management and direct patient engagement.

Emergency Response & Community Engagement

Assess and manage urgent medical issues during visits

Participate in community outreach and patient education initiatives

Qualifications

Masters Degree from an accredited NP program (Family or Adult/Geriatrics focus)

Current New York State NP license

National Board Certification (AANP ANCC or ONCC)

Due to patient population client is seeking providers who are bilingual in Russian or Chinese

Experience with EMR/EHR systems preferred

Strong clinical judgment communication skills and ability to work independently
