Job Type: Contract
Schedule
- 3 x 12-hour shifts per week
- Day Shift
- 36 guaranteed hours per week
- Holiday coverage required
Position Overview
Navitas Healthcare LLC is an experienced Licensed Practical Nurse (LPN) to provide high-quality care in a Long Term Care setting. The role requires strong clinical judgment independence and prior experience in skilled nursing or LTC environments.
Key Responsibilities
- Provide direct patient care including bathing feeding and toileting
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes promptly
- Maintain accurate and timely documentation
- Assist with care planning and MDS documentation (preferred)
- Ensure compliance with safety and infection control standards
- Collaborate with interdisciplinary healthcare teams
Required Qualifications
- Active unrestricted Colorado LPN License
- Minimum 3 years of Long Term Care (LTC) experience
- Current BLS/BCLS certification
Preferred Qualifications
- Experience with MDS documentation
- Experience using PointClickCare EMR
Skills & Competencies
