Job Summary

Must have valid drivers license
In order to fulfill the job functions listed below an individual must meet the following minimum qualifications.
Communicate effectively in person on the telephone and in writing.
Be Certified as a Nurse Aide and maintain State of Michigan Nurse Aide Certification and/or have the ability to
obtain certification within one year of hiring.
One-year long term care experience preferred.
Be in good standing on the Nurse Aide Register.
Complete Dementia care training.
Must obtain a Chauffeur License or State equivalent and maintain it in good standing for van transport.
Comply with all Facility policies and procedures.
Comply with all policies and procedures as may be established by your director.
Comply with all safety universal precautions and infection control policies and procedures.
Make independent decision when circumstances warrant such action.
Work harmoniously with other personnel.
Respect resident rights.
Present a friendly calm cooperative positive manner.
Understand the social psychological and recreational needs of the aged.
Implement goal-directed treatment plans following care plan recommendations.
To the best of your ability uphold the integrity of the activity in progress.
Follow direction as given by Director or acting supervisor.
On occasion may need to lift up to 40 lbs.
Prepare OBRA/State activity assessments.
Conduct individual and group resident activities such as crafts exercise cooking games gardening & bus trips.
Document daily resident activity participation.
Transport residents throughout the Facility and grounds.
Be involved with residents family members personnel and visitors under all conditions and circumstances.
Ability to drive facility van for resident outings appointments or other needs as assigned by management.
Post activity calendars and/or signs.
Requisition supplies as needed.
Maintain a clean orderly and safe work environment.
