Job Summary
Responsibilities
Under the supervision of a manager MSW or licensed clinician responsible for providing psychosocial-emotional support and services to patients and families. Assists patient and family in understanding coping and adapting to the personal emotional and bio-psychosocial issues. Provides comprehensive assessments and referrals crisis intervention and care management. Collaborates with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.
Qualifications
Education
Bachelors Degree in Social Science Social Work or related field required
Experience
1 year of relevant experience required
Licenses and Certifications
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
