Responsibilities include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of acute care experience in MS MS/Tele ICU and ER
- Care Coordination Utilization Review discharge planning Interqual and Epic experience
- Case Management experience in California
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of California
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
- Clinical Assessment Medication Administration and Treatment Planning skills
