Health Coordinator
Job Summary
About this role:
We are looking for a committed and enthusiastic person to join our Outreach team in partnership with Dartford Borough Council. The health coordinator role will work with people rough sleeping in Dartford improving access to services and facilities to reduce health inequalities in the borough. Working closely with a range of health care professionals - GPs mental health and community services and holistic support services to offer practical social and emotional support.
Situated within the community the role will facilitate connecting people with practical social and emotional support that will improve their health and wellbeing. This includes the management of volunteers.
The individuals accessing the service are likely to be entrenched street homeless people have complex support needs and have a history of non-engagement. If you are proactive have excellent communication and negotiation skills and want to support people to have a life beyond the streets then please get in touch.
You will be working as part of a wraparound holistic service. You will need a compassionate approach and the ability to work with each client to set clear goals and boundaries to empower them on a person-centred program of rehabilitation.

Hours: 30 per week
Working pattern: Working hours within 9am to 5pm Monday to Friday. Working days to be discussed upon appointment.
Contract type: Fixed Term until 31st March 2027
