Job Title:Assistant Professor in Ambulatory Care (Clinical Practice Track)

Job Summary: | This faculty member will practice as a member of multidisciplinary teams at the University of Colorado Family Medicine (50%) and Internal Medicine clinics (30%) with other academic responsibilities (20%) in teaching scholarship and service. These clinics are located in the Denver/Aurora metro regions of Colorado. The faculty member will collaborate to provide clinical coverage with other faculty colleague team members in internal medicine and family medicine clinics. This faculty member will provide exemplary care to patients and families further develop innovative clinical services for patients and providers and serve as a practice and training site. This position will provide clinical pharmacy services as a member of the interdisciplinary primary care team. Primary clinical responsibilities include developing and providing direct patient care clinical pharmacy services that focus on chronic disease management using collaborative drug therapy management protocols. Successful candidates will also interface with clinic providers to provide comprehensive medication management and other population health initiatives for patients. Clinical services and provider support will be delivered through a hybrid model that includes both in-person and telehealth/remote interactions with patients and addition to clinical practice successful candidates will be expected to provide experiential teaching for students in the Doctor of Pharmacy curriculum. The faculty member is expected to engage in scholarship that supports and advances their clinical and educational activities. Key Responsibilities: Function as an interdisciplinary member of the primary care teams to support all medication management activities. Establish collaborative working relationships with clinic providers and staff to become a valued and contributing team member. Work with clinic providers staff and administration to develop and implement clinic- and population-level medication interventions targeted to improve quality clinical metrics. Work collaboratively with providers clinic staff and care managers to reduce adverse drug events and improve medication management for care transitions and complex or high utilizing patients. Educate staff and providers regarding medication management best practices including updates on new medications and clinical guidelines chronic disease state management and others. Provide quality teaching services in the School of Pharmacy Doctor of Pharmacy program. Teaching includes precepting student Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs) evaluating student performance using various assessment methods and mentoring and advising students. Opportunities to prepare and present lectures and to facilitate active learning sessions are also available. Produce scholarly work that supports clinical practice and/or educational activities including peerreviewed publications and research demonstrating the value of clinical pharmacy services. Participate in professional service activities such as university or school committee work participation in professional organizations and attendance at seminars conferences special courses and scientific meetings.

Qualifications: | Minimum Qualifications: Applicants must meet minimum qualifications at the time of hire. Applicants must have a PharmD degree Completed an ASHP-accredited PGY2 residency in ambulatory care (or a similar program) or completed an ASHP-accredited PGY1 plus 2 years of clinical experience; or equivalent work experience in ambulatory care. Candidates must have or be eligible for pharmacist licensure in Colorado.
