Occupational Therapist

• Provide ministry-funded community-based occupational therapy services in clients homes and/or through virtual care
• Conduct occupational therapy assessments using interviews standardized tools and informal assessment techniques including:
  • Home safety and home modifications
  • Equipment prescription
  • Fall prevention
  • Mobility and physical functioning
  • Cognition
  • Stroke rehabilitation and chronic conditions
• Develop and implement evidence-based intervention plans in collaboration with clients and caregivers
• Assign supervise and monitor occupational therapy assistants as appropriate
• Educate clients and caregivers and connect them with relevant community agencies and resources
• Complete funding applications when required
• Maintain accurate timely and professional documentation including:
  • Assessment findings
  • Care plans
  • Progress and discharge summaries
  • Reports aligned with referral source requirements and COTO standards

Qualifications :

• Registration in good standing with the College of Occupational Therapists of Ontario (COTO)
• Ability to work independently within a community-based model
• Strong communication documentation and clinical reasoning skills
• Experience in community practice is an asset but new graduates are welcome
• Valid G or G2 drivers license with access to a reliable vehicle for community travel
• Proficiency with computer applications including MS Office Suite and proprietary clinical software
