Registered Advanced Nurse Practitioner (RANP) /Candidate Advanced Nurse Practitioner (cANP)

Critical Care Outreach - Adult Nursing Service

Full Time Permanent Post

PURPOSE OF THE ROLE
The overall purpose of the post is to provide safe timely evidenced based nurse-led care to patients at an advanced nursing level. This involves undertaking and documenting a complete episode of patient care (assess plan treat and discharge patients) according to collaboratively agreed protocols and scope of practice in the clinical setting; demonstrating advanced clinical and theoretical knowledge critical thinking clinical leadership and decision making skills.

The ANP/ANP c will provide a critical care consultative educational and direct clinical intervention approach to support staff caring for deteriorating patients in Adult admitted clinical areas. The ANP/cANP will liaise with anaesthetic consultant on duty in ICU/PACU the ICU registrar and primary teams identifying patients of concern of clinical deterioration. Further referrals can be received through calls of concern and part of the hospital escalation policy of patients scoring high INEWS by any healthcare team member. The ANP/cANP will liaise with Nurse Site Managers medical registrar on call nursing teams on ward areas to identify patients at risk of deterioration complete assessment assist in developing management plan and evaluation of the interventions.

The ANP/cANP will participate in the clinical handover of patients with the other members of the Critical Care Outreach team Medical Registrar on call at night Critical Care Anaesthetic consultant and registrar at change of shift.

The ANP/ cANP will participate as a key member of the Emergency Response Team and the Cardiac Arrest Team.

QUALIFICATIONS

1. Applying as a RANP
  • Be registered on the Advanced Nurse Practitioner Division of the active Register held by Nursing and Midwifery Board of Ireland
Or
  • Be eligible to register in the Advanced Nurse Practitioner Division of the Nursing and Midwifery Board of Ireland Register by meeting the criteria for registration as an Advanced Nurse Practitioner as specified
And
  • Have a broad base of clinical experience relevant to the advanced field of practice - (Critical Care Outreach)
And
  • Demonstrates the competencies relevant to the specialist area of advanced practice.
  • Possesses the requisite knowledge and ability including a high standard of suitability and clinical professional and administrative capacity to properly discharge the functions of the role.
  • Practitioners must maintain active annual registration on the Advanced Nurse Practitioner Division of the register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland for the role.

Applying as a Candidate /ANP
  • Be registered in the General Division of the active Register held by Nursing and Midwifery Board of Ireland or be entitled to be so registered.
And
  • Have a broad base of clinical experience relevant to the advanced field of practice

And
  • Be eligible to undertake a Masters Degree (or higher) in Nursing or a Masters Degree which is relevant or applicable to the advanced field of practice. The Masters programme must be at Level 9 on the National Framework of Qualifications (Quality & Qualifications Ireland) or equivalent. Educational preparation must include at least three modular components pertaining to the relevant area of advanced practice in addition to clinical practicum.
Or
  • Be currently undertaking a Masters Degree in Nursing (Advanced Practice Pathway) or be eligible to register to undertake additional Level9 National Framework of Qualifications (Quality and Qualifications Ireland) specific modules of a Masters Degree in Nursing (Advanced Practice Pathway) within an agreed timeframe. Educational preparation must include at least three modular components pertaining to the relevant area of advanced practice in addition to clinical practicum.
Or
  • Possess a Masters Degree (or higher) in Nursing or a Masters Degree which is relevant or applicable to the advanced field of practice. The Masters programme must be at Level 9 on the National Framework of Qualifications (Quality & Qualifications Ireland) or equivalent. Educational preparation must include at least three modular components pertaining to the relevant area of advanced practice in addition to clinical practicum.
And
  • Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical professional and administrative capacity to properly discharge the functions of the role.

2. Age Restrictions In Relation To Applications
Age restriction shall only apply to a candidate where they are not classified as a new entrant (within the meaning of the Public Service Superannuation (Miscellaneous Provisions) Act 2004). A candidate who is not classified as a new entrant must be under 65 years of age on the first day of the month in which the latest date for receiving completed applications for the office occurs.

3. Health
A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health as would indicate a reasonable prospect of ability to render regular and efficient service.

5. Garda Vetting
Garda Vetting is sought for all employees and prospective employees of Tallaght University Hospital. Given the specialised nature of the services provided your appointment will be subject to satisfactory Garda Vetting and re-vetting in circumstances where the Hospital deems it appropriate. The Hospital will then process the necessary documentation to endeavour to obtain satisfactory Garda clearance for you. You are obliged to disclose previous and any criminal convictions acquired during the course of your employment. Should the Hospital obtain information from the Garda Vetting Unit to indicate that your Garda clearance report is not satisfactory and / or if you have supplied the hospital with false or misleading information in relation to your Garda clearance status the Hospital reserves the right to withdraw or terminate this contract in accordance with Tallaght University Hospital Garda Vetting policy.

6. Mandated and Designated Persons under Children First Act 2015
Schedule 2 of the Children First Act 2015 specifies the classes of persons defined as Mandated Persons for the purposes of the Act. This includes a range of disciplines that are employed by the hospital including all medical practitioners registered nurses physiotherapists psychologists social workers and others. This includes staff working in adult services. All mandated persons have two main legal obligations under the Children First Act 2015. Mandated persons under the legislation are required to report any knowledge belief or reasonable suspicion that a child has been harmed is being harmed or is at risk of being harmed. The Act defines harm as assault ill-treatment neglect or sexual abuse and covers single and multiple instances. Mandated persons are also required to assist Tusla if requested in assessing a concern which has been the subject of a mandated report. Designated Officer are required to receive reports of suspected child abuse or neglect from any person who is of the opinion that (a) a child has been or is being assaulted ill-treated neglected or sexually abused or (b) a childs health development or welfare has been or is being avoidably impaired or neglected. PARTICULARS OF OFFICE

1. The appointment is full time permanent and pensionable.

2. Remuneration:
Remuneration is in accordance with the salary scale approved by the Department of Health & Children. Current remuneration with effect from 1st February 2026 is:

Candidate Advanced Nurse Practitioner 71432 by 6 Increments to 80671 per annum
Advanced Nurse Practitioner (General) 72122 by 9 increments to 89004 per annum

Salary scales will be updated in line with nationally agreed pay agreements and will be updated accordingly and retrospective payments applied as applicable.

3. Pension arrangements and retirement age
There are three superannuation schemes currently in operation for staff in Tallaght University Hospital:

(a) Voluntary Hospital Superannuation Scheme (Non-New Entrant)
(b) Voluntary Hospital Superannuation Scheme (New Entrant)
(c) Single Public Service Pension Scheme

Please read carefully the summary of the criteria for the three superannuation schemes below. This will assist you in identifying which scheme membership is applicable to your employment circumstances.

(a) If you have been continually employed in the public service prior to the 1st April 2004 and you have not had a break in service of more than 26 weeks you will be included in the Voluntary Hospital Superannuation Scheme (Non-New Entrant) with a Minimum Retirement Age of 60 and a Maximum Retirement Age of 65.

(b) If you have been continually employed in the public service post the 1st April 2004 and you have not had a break in service of more than 26 weeks you will be included in the Voluntary Hospital Superannuation Scheme (New Entrant) with a Minimum Retirement Age of 65. There is no Maximum Retirement Age.

(c) If you have commenced employment in the public service as a new entrant or you are a former public servant returning to the public service after a break of more than 26 weeks you will be included in the Single Public Service Pension Scheme effective from the 1st January 2013 with a minimum retirement age of 66 (rising to 67 and 68 in line with state pension changes). The maximum retirement age under this scheme will be age 70.

4. Duties:
The ANP/ANP c will perform such duties as are outlined in this job description.

5. Hours of work:
Normal working hours are 37.5 worked over 7 days. Your contracted hours of work are liable to change between the hours of 8 am to 8 pm over 7 days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement.

6. Probation:
The successful candidate will be appointed initially for a probationary period of 6 months. During the probationary period progress or otherwise will be monitored and at the end of the probationary period the service will (a) be certified as satisfactory and confirmed in writing or if not satisfactory the probationary period may be extended by 3 months.

7. Annual Leave:
Annual leave entitlement is 25-28 working days (pro rata) depending on length of service plus 10 Bank Holidays per annum as they occur. The annual leave year runs from 1st of April to 31st March each year.

8. Sick Leave:
Payment of salary during illness will be in accordance with arrangements as approved from time to time by the Department of Health and Children.

9. Termination of Office:
The employment may be terminated at any time by 3 months notice on either side except where circumstances are dictated by the Minimum Notice and Terms of Employment Act 1973/91. The Managements right under this paragraph shall not be exercised save in circumstances where the Management is of the opinion that the holder of the office has failed to perform satisfactorily the duties of the post or has misconducted himself/herself in relation to the post or is otherwise unfit to hold the appointment.

SUMMARY OF ROLE
The advanced practice service is provided by nurses who practice at a higher level of capability as independent autonomous and expert advanced practitioners. The overall purpose of the service is to provide safe timely evidenced based nurse-led care to patients at an advanced nursing level .This involves undertaking and documenting complete episodes of patient care (In medical Notes) which includes comprehensively assessing audits planning treating and discharging patients in accordance with collaboratively agreed caseloads local policies procedures protocols and guidelines and/or service level agreements/ memoranda of understanding.

The cANP/ RANP Critical Care Outreach demonstrates advanced clinical and theoretical knowledge critical thinking clinical leadership and complex decision-making abilities.

The cANP/ RANP Critical Care Outreach practices in accordance with the Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (NMBI 2021) the Scope of Nursing and Midwifery Practice Framework (NMBI 2015) Advanced Practice (Nursing) Standards and Requirements (NMBI 2017) and the Values for Nurses and Midwives in Ireland (Department of Health 2016).

The cANP/ RANP Critical Care Outreach service provides clinical leadership and professional scholarship in the delivery of optimal nursing services and informs the development of evidence based health policy at local regional and national levels.

The cANP/ RANP Critical Care Outreach contributes to nursing research audits and key performance indicators that shapes and advances nursing practice education and health care policy at local national and international levels.

The ANP/cANP will develop the job description and supporting documentation under the direction of the Health Care Providers Advanced Practice Stakeholder Governance Group to enable the individual nurse to meet the NMBI Criteria for Registration as an Advanced Nurse Practitioner as set out in Advanced Practice (Nursing) Standards and Requirements (NMBI 2017).

Professional Practice portfolio
The cANP/ RANP Critical Care Outreach must maintain a professional practice portfolio incorporating evidence of learning from continuing professional development clinical supervision reflective practice and review of his/her own scope of practice in accordance with regulatory requirements and service need.

Role and Responsibilities
The cANP/ RANP Critical Care Outreach practices to a higher level of capability across six domains of competence as defined by Bord Altranais agus Cnáimhseachais na hÉireann Advanced Practice (Nursing) Standards and Requirements (NMBI 2017).

The cANP/ RANP Critical Care Outreach will be required to apply ethically sound solutions to complex issues related to individuals and populations by:
  • Demonstrating accountability and responsibility for professional practice as a lead healthcare professional in the care of deteriorating patients and critical care patients.
  • Collaborating with his/her/ their supervisor and local stakeholder group to scope the caseload and scope of practice for the cANP/ RANP Critical Care Outreach

  • Conduct a comprehensive holistic health assessment using evidenced based frameworks policies procedures protocols and guidelines to determine diagnoses and inform autonomous advanced nursing care
  • Synthesise and interpret assessment information particularly history including prior treatment outcomes physical findings and diagnostic data to identify normal at risk and subnormal states of health

  • Implementing practice changes using negotiation and consensus building in collaboration with the multidisciplinary team and persons receiving care.

The cANP/ RANP Critical Care Outreach will lead in multidisciplinary team planning for transitions across the continuum of care by enhancing his/her/their competence and capability to:
  • Demonstrate clinical leadership in the design and evaluation of services (for example findings from research audit metrics new evidence)
  • Engaging in health policy development implementation and evaluation (for example key performance indicators from national clinical and integrated care programme/HSE national service plan/ local service need to influence and shape the future development and direction of advanced practice in Critical Care Outreach
  • Identifying gaps in the provision of care and services pertaining to his/her/their area of advanced practice and apply the best available evidence
  • Completion of relevant training and availing of supports in the Leadership Academy including coaching mentoring and leadership development as applicable.

Below is a list of current competencies to the cANP/RANP Critical Care Outreach this will develop further in line with expanding caseload and expanding scope of practice.
  • Demonstrate in-depth knowledge of Critical Care Sepsis Advanced Life Support recognising and managing deteriorating patients in the acute hospital setting.
  • Demonstrate advance skills assessing and managing deteriorating patients with critical care requirements.
  • ABG interpretation and management specific to the advanced clinical knowledge required for cANP/RANP and progress to ABG sampling in line with scope of practice.
  • Demonstrate ability to interpret clinical laboratory data in the context of the case.
  • Demonstrate advanced skills performing and reporting on critical care procedures
  • Demonstrate advance skills assessing deteriorating patients and managing care requirements in collaboration with the multidisciplinary team.
  • Demonstrate in-depth knowledge of the principles of administration of Non Invasive
  • Ventilation Airvo and other respiratory therapies for respiratory conditions including the

Performance Management & Evaluation
  • Performance Indicators are required to evaluate nursing interventions and implement initiatives to improve the quality and quantity of nursing care provided. Performance indicators should have a clinical nursing focus as well as a breakdown of activity including patients seen and addition Performance Indicators should identify areas of good practice that must be recognised and celebrated (HSE 2015).
  • The Department of Health (2017) Framework for National Performance Indicators for Nursing and Midwifery provides a guiding framework for the development of Nursing and Midwifery Performance Indicators.
  • In collaboration with the Assistant Director of Nursing the cANP/ RANP Critical Care Outreach will identify and develop Nursing Performance Indicators for their area of practice collect and collate data which will provide evidence of the impact and effectiveness of the interventions undertaken.
  • Participate in clinical audit and evaluate audit results and research findings to identify areas for quality improvement in collaboration with nursing and multidisciplinary team colleagues (primary and secondary care).
  • Participate in clinical audit symposiums grand round and provide an Annual Service Report to the Perioperative Directorate Nurse Manager and the Director of Nursing.
  • Develop the Critical Care outreach service in collaboration with the Perioperative Directorate Nurse Manager and the multidisciplinary team and in line with corporate strategy as well as service improvement from clinical audits and service evaluations.

Management and Leadership
  • Support the principle that person-centred care comes first at all times and will approach the effective efficient and resourceful planning organisation and delivery of cANP/ RANP Critical Care Outreach nursing service with the flexibility and enthusiasm necessary to make this principle a reality for every patient.

  • Comply and enact Health and Safety responsibilities as outlined in Hospital policies protocols and procedures relevant to your area.
  • Prepare risk assessments and departmental safety statements as required.

Hygiene/Infection Control
  • Ensure you are aware of your responsibility for Hygiene awareness. Hygiene is defined as The practice that serves to keep people and environments clean and prevent infection.
  • Act as a role model and actively promote infection control and prevention and ensure that staff in your area familiarise themselves with the hospital infection control policies and guidelines as outlined in the Infection Control Manual.
  • Ensure that staff in your area are aware that they work in an area where there is potential for transmission of infection.
  • Ensure that all your staff are advised that they have a responsibility to prevent the transmission of infection particularly in relation to hand hygiene.

Quality safety and risk management
  • Support the delivery of the Quality Safety and Risk Management Programme including the appropriate identification reporting and management of risks and incidents throughout the hospital.

Confidentiality
  • You will be aware of the confidential nature of Hospital work and in particular the right of patients to confidentiality. To this end you will not discuss workings of the Hospital or its patients or disclose any information of a confidential nature except as required to do so in the course of your work. No records documents or property of the Hospital may be removed from the premises of the Hospital without prior authorisation. You must return to the Hospital upon request and in any event upon the termination of your employment all documents or other property of the Hospital which are in your possession or under your control.

Data Management
  • Ensure compliance with the obligations required by the Data Protection Act 2018.


NOTE: The extent and speed of change in the delivery of health care is such that adaptability is essential at this level of appointment. The appointee will be required to maintain enhance and develop their knowledge skills and aptitudes necessary to respond to this changing environment. They will also be required to participate in and support the Hospitals Digital transformation strategy which may impact work processes and role profiles in the future. The Job Description must therefore be regarded as an outline of the major areas of accountability at the present time which will be reviewed and assessed on an on-going basis as advancements and developments evolve.
