The Registered Nurse provides skilled nursing care case management and hospice care to patients in Spokane County WA and surrounding areas. This role involves a mix of in-office duties including admissions triage and phone calls along with patient visits in their homes. The RN will work within an interdisciplinary team to deliver comprehensive care ensuring patient comfort and quality of life.

Location:Spokane Valley Washington United States
Responsibilities:
- Conduct patient assessments to determine care needs.
- Administer medications and treatments as prescribed.
- Develop and implement patient care plans in collaboration with the interdisciplinary team.
- Provide skilled nursing care to patients in their homes.
- Perform admissions triage and phone consultations.
- Document patient care accurately and efficiently using EPIC/HCHB.
- Maintain effective communication with patients families and the healthcare team.
- Ensure compliance with all regulatory and organizational policies and procedures.
- Manage a patient caseload optimizing visit schedules to minimize travel time.

Required Skills & Certifications:
- Active Registered Nurse license in the state of Washington.
- BLS certification.
- Minimum of 1 year of skilled hospice experience.
- HCHB experience.
- Valid drivers license car insurance and reliable transportation.
- Active Registered Nurse license in the state of Washington.

- Home Care Home experience.
- Home Care Home experience.
Special Considerations:
- Traveler is expected to have their own transportation.
- The system does a good job of grouping clients minimizing drive and windshield time. Occasional maximum radius of 50 miles.
- The facility can call off UP TO one shift per pay period (every two weeks).
- Traveler wont have a specific case load but will be utilized for admits triage phone calls RN visits etc. They will be in the office unless out doing visits.
- Traveler is expected to have their own transportation.

- 7 on / 7 off schedule.
- 10-hour shifts Friday through Thursday from 12:00 PM to 10:30 PM.
