Job Summary

1. Under the direct supervision of a physical therapist carries out individualized treatment plan as evidenced by clinical documentation and in compliance with federal and state regulations organizational and departmental policies and standards of care and APTA guidelines. 2. In collaboration with the Physical Therapists assists in the development of a plan of care to promote optimal functional independence and safety as documented by clinical record in compliance with federal and state regulations organizational and departmental policies and standards of care and APTA guidelines. 3. Plans for and provides skilled physical therapy treatment including but not limited to restorative and maintenance therapeutic techniques patient/care giver education etc. assessing the patient s status and progress on each visit and ensuring quality of care as documented in the clinical record and in compliance with federal and state regulations organizational and departmental policies and standards of care and APTA guidelines. 4. Assists in the coordination patient care with other clinical team members including but not limited to physicians nurses care givers physical therapists other rehab services and students ensuring optimal patient care and communication as noted in the clinical record and in compliance with federal and state regulations organizational and departmental policies and standards of care and APTA guidelines. 5. Actively remains vigilant to identify and report changes in patient conditions related to prescribed plan of care and takes action to report changes in patient status to case manager in a timely manner. 6. Actively communicates on-going clinical and operational activities with Physical Therapists to maximize overall quality of care. 7. Completes and coordinates required paperwork in compliance with federal and state regulations organizational and departmental policies and standards of care and APTA guidelines to ensure that documentation reflects care provided and the outcomes achieved as evidenced in the clinical record. 8. Maintains and enhances personal knowledge as well as promotes standards in physical therapy. 9. Ability to stay up-to-date on HHCAHPS standards and to integrate standards into visit protocol.
