Job Overview

Key Responsibilities
- Conduct psychosocial assessments of patients and families
- Develop and implement individualized discharge plans
- Coordinate post-acute care services referrals and community resources
- Collaborate with physicians nurses and interdisciplinary teams
- Support patients with emotional social and financial challenges
- Manage a caseload of approximately 2023 patients
- Ensure accurate and timely documentation of social work interventions
- Advocate for patient needs within the healthcare system
- Participate in discharge planning rounds and care coordination meetings
- Help reduce barriers to care and improve patient outcomes
Qualifications
- Masters Degree in Social Work (MSW) required
- Minimum 1 year acute care hospital social work experience required
- Strong knowledge of discharge planning and care coordination
- Excellent communication and interpersonal skills
- Ability to manage high-volume caseloads in a fast-paced environment
- Strong documentation and organizational skills
