Job Description:
We are seeking an experienced Medical Social Worker (MSW) to support patient care in an acute care setting. The ideal candidate will play a key role in care coordination discharge planning and providing psychosocial support to patients and their families.
Key Responsibilities:
- Provide psychosocial assessments and support to patients and families
- Assist with discharge planning and coordination of post-acute care services
- Collaborate with interdisciplinary healthcare teams
- Manage a caseload of approximately 2023 patients
- Connect patients with community resources and support services
- Maintain accurate and timely documentation
Required Experience & Skills:
- Minimum 1 year of acute care social work experience
- Prior travel experience required
- Strong knowledge of care coordination and discharge planning
- Ability to manage multiple cases in a fast-paced environment
- Excellent communication and interpersonal skills
Qualifications:
- Masters Degree in Social Work (MSW) Required
- Active state license (preferred; must be active by start date if required)
- Strong organizational and problem-solving skills
