Job Overview
The Licensed Practical Nurse (LPN) in a long-term care setting provides direct patient care to residents ensuring their safety comfort and overall well-being. The LPN works under the supervision of a Registered Nurse and collaborates with the healthcare team to deliver high-quality patient-centered care.
Key Responsibilities
- Provide direct nursing care to residents in a long-term care facility
- Administer medications and treatments as prescribed
- Monitor and record vital signs and patient conditions
- Assist residents with activities of daily living (ADLs)
- Perform wound care and other clinical procedures
- Maintain accurate and timely patient documentation
- Report changes in patient condition to the RN or physician
- Ensure compliance with infection control and safety protocols
- Support care plan implementation and updates
- Respond promptly to patient needs and emergencies
Qualifications
Education
- Diploma or certification from an accredited Practical Nursing program
or High School Diploma
Licensure and Certification
- Active LPN/LVN License (required)
- Basic Life Support (BLS) certification (required)
Experience and Requirements
- Minimum 1 year of LPN experience in long-term care or skilled nursing facility preferred
- Familiarity with EMR systems
- Ability to work flexible shifts including nights weekends and holidays
- Knowledge of geriatric care and chronic conditions
- Strong time management and organizational skills
- Ability to work in a fast-paced environment
- Team collaboration and adaptability
