Shift: Day Shift (Variable 12-hour shifts)
Schedule: 36 hours/week
Position Summary
Navitas Healthcare LLC isa skilled and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) team. This role provides the opportunity to deliver quality nursing care in a supportive and resident-focused environment.
Key Responsibilities
- Provide direct nursing care to residents including medication administration and treatments as prescribed
- Monitor document and report resident conditions vital signs and progress
- Assist in developing and implementing individualized care plans
- Collaborate with RNs CNAs and other healthcare team members
- Maintain compliance with facility policies procedures and state regulations
- Educate residents and families regarding care plans wellness and health maintenance
- Participate in quality improvement initiatives to improve resident outcomes
- Maintain accurate and timely documentation
Qualifications
- Active LPN license in South Dakota
- Experience in Long-Term Care preferred
- Strong clinical judgment and critical thinking skills
- Good communication and teamwork abilities
