Position Title: Licensed Practical Nurse (LPN) Long-Term Care (LTC) Location: Thorp WI 54771 Employment Type: Full-Time (40 hours/week) Shift: Evenings (8-hour shifts)
Position Overview
Navitas Healthcare LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) facility. The LPN will provide direct patient care to residents ensuring their safety comfort and overall well-being. This role works under the supervision of a Registered Nurse (RN) and collaborates closely with the healthcare team to deliver high-quality patient-centered care.
Key Responsibilities
Provide direct nursing care to long-term care residents
Administer medications and treatments as prescribed
Monitor record and report vital signs and patient conditions
Assist residents with Activities of Daily Living (ADLs)
Perform wound care and other clinical nursing procedures
Maintain accurate and timely patient documentation
Report any changes in patient condition to RN or physician
Follow infection control and facility safety protocols
Support implementation and updates of individualized care plans
Respond promptly to resident needs and emergency situations
Required Qualifications
Education:
Diploma or certification from an accredited Practical Nursing program (or equivalent educational qualification)
Licensure/Certification:
Active LPN/LVN license (state of practice)
Current Basic Life Support (BLS) certification required
Experience:
Minimum 1 year of LPN experience in Long-Term Care or Skilled Nursing Facility preferred
Experience with Electronic Medical Records (EMR) systems preferred
Ability to work flexible schedules including weekends and holidays
