Position Title: Licensed Practical Nurse (LPN) Long-Term Care (LTC)

Employment Type: Full-Time (40 hours/week)
Shift: Evenings (8-hour shifts)
Position Overview
Navitas Healthcare LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) facility. The LPN will provide direct patient care to residents ensuring their safety comfort and overall well-being. This role works under the supervision of a Registered Nurse (RN) and collaborates closely with the healthcare team to deliver high-quality patient-centered care.
Key Responsibilities
- Provide direct nursing care to long-term care residents
- Administer medications and treatments as prescribed
- Monitor record and report vital signs and patient conditions
- Assist residents with Activities of Daily Living (ADLs)
- Perform wound care and other clinical nursing procedures
- Maintain accurate and timely patient documentation
- Report any changes in patient condition to RN or physician
- Follow infection control and facility safety protocols
- Support implementation and updates of individualized care plans
- Respond promptly to resident needs and emergency situations
Required Qualifications
Education:
- Diploma or certification from an accredited Practical Nursing program (or equivalent educational qualification)
Licensure/Certification:
- Active LPN/LVN license (state of practice)
- Current Basic Life Support (BLS) certification required
Experience:
- Minimum 1 year of LPN experience in Long-Term Care or Skilled Nursing Facility preferred
- Experience with Electronic Medical Records (EMR) systems preferred
- Ability to work flexible schedules including weekends and holidays
