A well-established patient-centered healthcare organization in the New Haven area is seeking a full-time Family Medicine Nurse Practitioner to join its growing outpatient practice. The Nurse Practitioner will provide comprehensive primary care services across the lifespan including preventive care chronic disease management and acute care visits. Responsibilities include conducting physical exams diagnosing and treating common conditions prescribing medications developing patient-centered care plans and collaborating closely with physicians nurses and support staff while utilizing EMR systems and participating in quality improvement initiatives.
