Pathways RN Facilitator

Hospice of Marion County a member of Empath Health is seeking a Pathways RN Facilitator who works directly with community partners out of the Empath Health service area to ensure smooth facilitation of patient transfers/travelers to our service area and optimal access to the Empath Health Integrated Network of Care.

Shift: Monday Friday 8am to 5pm.

What youll Do

• Collaborate with out-of-area partners to facilitate smooth patient transfers and appropriate care coordination.
• Serve as a key resource and advocate for patients families and care partners throughout the admission and transition process.
• Partner with internal departments including Finance Reimbursement and Interdisciplinary Teamsto ensure continuity of care.
• Maintain compliance with hospice regulations and documentation standards within the EMR.
• Participate in ongoing review and improvement of Care Navigation processes promoting Empath Healths mission and values.

What Youll Need

• Education and/or Experience: Minimum two (2) years of hospice experience.
• Licensure: Current valid Florida RN license.
• Continuing education: As required for licensure.
• Knowledge of hospice regulations such as admission/qualification criteria reimbursement and levels of care preferred.
• Excellent positive relationship building outreach efforts problem solving organizational time management creativity interpersonal skills written and verbal communication a must.
