Title: RN Wound Care

Job Summary
The RN Wound Care is responsible for delivering specialized care to patients with wound ostomy and continence needs. This role involves comprehensive assessments care planning treatment recommendations and ensuring optimal patient outcomes.
Key Responsibilities
- Conduct health assessments focused on wound ostomy and continence care
- Perform physical assessments and develop individualized care plans
- Recommend appropriate medications and treatment strategies
- Develop revise and evaluate standards of care
- Assist in selecting appropriate supplies equipment and technology
- Assess and document patient responses to treatments (e.g. wounds fistulas ostomies)
- Evaluate quality and effectiveness of care delivered by clinical staff
- Collaborate with interdisciplinary teams and initiate referrals as needed
- Educate patients families and staff on wound care and prevention strategies
- Develop and deliver staff education programs including orientation and continuing education
- Conduct pressure injury risk assessments and implement preventive measures
- Escalate clinical ethical or conflict-related concerns when necessary
- Partner with leadership to recommend and implement best practices
- Participate in quality improvement initiatives (e.g. incidence/prevalence studies nursing quality programs)
- Ensure culturally competent care and education
- Maintain patient confidentiality and data security standards
- Identify and coordinate community resources for patients
- Manage specialty equipment (e.g. specialty beds negative pressure devices)
Required Qualifications
Education:
- Associate Degree in Nursing (ASN/ADN) from an accredited program
Experience:
- Minimum 2 years of Medical-Surgical (Med/Surg) experience
- Experience in wound care and colostomy care
Licensure & Certifications:
- Active New York State RN License
- NYSED Primary Source Verification
- AHA BLS Certification
Required Certification (One of the Following):
- CWOCN Certified Wound Ostomy Continence Nurse
- CWON Certified Wound Ostomy Nurse
- CWCN Certified Wound Care Nurse
- CWS Certified Wound Specialist
- WCC Wound Care Certified
