Job Summary

Registered Nurse Diabetes Management / Diabetes Educator

Stapleford Medical Clinic is seeking a skilled and compassionate Full-Time (37.5 hrs. weekly) Registered Nurse Diabetes Management / Diabetes Educator to provide high-quality patient-centred care through comprehensive education clinical management and ongoing monitoring for patients with prediabetes Type 1 and Type 2 diabetes. This role focuses on empowering patients to effectively manage their condition through evidence-based education lifestyle counselling medication support and continuous follow-up. The position requires strong clinical expertise in diabetes care excellent communication and coaching skills and the ability to build trusting relationships that support long-term behaviour change and improved health outcomes.

This position is part of a newly developed Innovation Fund between the Saskatchewan Government and medical clinics across Saskatchewan with the aim of improving patient access and quality care through team-based support that will deliver better access to primary care in the province of Saskatchewan. This position is temporary until March 31 2027 with the possibility of an extension.

Responsibilities:

  • Develop implement and update diabetes care protocols and work standards in collaboration with Physician Lead(s) and the care team.
  • Conduct comprehensive assessments for patients with diabetes or at risk of developing diabetes including medical history lifestyle factors and risk stratification.
  • Provide individualized diabetes education including:
  • Blood glucose monitoring and interpretation
  • Insulin administration and medication management
  • Nutrition counselling and carbohydrate counting
  • Physical activity guidance
  • Prevention and management of hypoglycemia and hyperglycemia
  • Support initiation and titration of diabetes medications (including insulin) as per established clinical protocols and scope of practice.
  • Monitor and manage chronic disease indicators (e.g. A1C blood pressure lipids) and adjust care plans accordingly in collaboration with providers.
  • Provide ongoing follow-up and coaching to support patient adherence self-management and lifestyle modifications.
  • Collaborate with physicians dietitians pharmacists and other healthcare professionals for integrated care planning and case management.
  • Identify and address barriers to care including social determinants of health and facilitate referrals to appropriate community resources.
  • Deliver group education sessions or workshops on diabetes prevention and management (if applicable).
  • Support screening and early intervention for prediabetes and metabolic syndrome.
  • Maintain accurate timely and detailed documentation in the Electronic Medical Record (EMR) system.
  • Apply infection prevention and control (IPAC) standards in all clinical and educational interactions.
  • Participate in team huddles case conferences and quality improvement initiatives related to chronic disease management.
  • Stay current with best practices clinical guidelines and advancements in diabetes care through ongoing professional development.

Qualifications:

  • Current registration and good standing with the College of Registered Nurses of Saskatchewan (CRNS).
  • Bachelor of Science in Nursing (BScN) or equivalent nursing degree.
  • Current Basic Life Support (BLS) certification (ACLS an asset).
  • Minimum 23 years of clinical nursing experience preferably in primary care or chronic disease management.
  • Demonstrated experience in diabetes care and patient education.
  • Certified Diabetes Educator (CDE) designation is strongly preferred.
  • Strong knowledge of diabetes pathophysiology pharmacological treatments and lifestyle management strategies.
  • Experience with insulin initiation and titration protocols (asset).
  • Proficiency with EMR systems (e.g. Accuro or equivalent).
  • Excellent communication coaching and motivational interviewing skills.
  • Ability to work both independently and collaboratively within an interdisciplinary team.
  • Strong organizational skills with the ability to manage a caseload and prioritize effectively.
  • Demonstrated commitment to patient-centered care and cultural sensitivity.
