We are seeking a Case Management Supervisorto lead and support our case management teamensuring exceptional care coordination compliance and service delivery for our members.

What Youll Do
Lead coach and develop a team of Case Managers
Overseeclient assessments service planning and care coordination
Ensure quality compliance and accurate documentation
Monitor team productivity performance and outcomes
Conduct home visits and support care plan implementation
Review member chartsand ensure readiness for billing and authorization
Partner with leadership to improve processes and drive outcomes
Support recruitment training and onboarding of team members
Build relationships with clients families and referral sources
What Were Looking For
Active RN or LPN license (required)
Bachelors degree preferred (or equivalent experience)
2 years of experience working with elderly or disabled populations
Leadership or supervisory experience preferred
Strong communication organization and team leadership skills
Knowledge of home care case management and compliance standards
Ability to manage performance productivity and quality metrics
Reliable personal transportation (travel required 15%)
