The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and team members are essential.

Location:Healdsburg California United States
Responsibilities:
- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management.
- Provide hospice care.
- Collaborate with patients providers and other team members.
- Float to lower acuity units including MS/Tele and MS.
- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management.
- Provide hospice care.
- Collaborate with patients providers and other team members.
- Float to lower acuity units including MS/Tele and MS.
Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum of 2 years of hospital home health or hospice care experience (5 years preferred)
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in CA
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
