Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration with patients providers and team members are essential. The position requires experience in telemetry and medsurg and the ability to float between units as needed.

Responsibilities:
- Provide skilled nursing care to patients.
- Perform patient assessments and monitor vital signs.
- Administer medications and treatments as prescribed.
- Develop and implement patient care plans.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Document patient care accurately and timely.
- Float between units based on unit needs.
- Provide skilled nursing care to patients.
- Perform patient assessments and monitor vital signs.
- Administer medications and treatments as prescribed.

Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum of two years of hospital home health or hospice care experience.
- Must be trained in telemetry and medsurg.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
- Active Registered Nurse license in the state of working.

- Must be willing to work every other weekend.
- No block schedules are available.
- Must not be a first-time traveler.

- The expected ratios are 1:4 for telemetry and 1:5 for medsurg.
- The position requires working every other weekend.
- No on-call or standby is required.
- 48-hour shifts are not approved.
