Job Summary

SH- 889 Certified Nurse Practitioner (Senior)

Position Overview:
This advanced practice position works under the general direction of the Clinical Medical Lead. Responsibilities include performing patient care as a nurse practitioner including physical examinations ordering diagnostic tests prescribing medications and developing care plans. The Certified Nurse Practitioner is an integral member of the healthcare team contributing to high-quality patient-centered care in a clinical or public health setting.

Key Responsibilities

  • Perform comprehensive patient care as a nurse practitioner with limited or no on-site access to a physician for guidance and oversight.
  • Perform physical examinations obtain medical histories and record findings in a systematic accurate and succinct manner.
  • Order perform and interpret routine laboratory and diagnostic tests; review results and make appropriate plan of care changes.
  • Prescribe appropriate medications including controlled substances within the guidelines of the practice agreement and state regulations.
  • Develop and implement care plans for health promotion health maintenance disease prevention and provision of health education and counseling.
  • Provide clinical management of patients and co-manage care for patients with selected health complications in collaboration with supervising physician.
  • Perform or direct others to perform nursing assessments lab tests measurements and case management activities.
  • Arrange referrals to other members of the healthcare team community providers and community resources as needed.
  • Document all pertinent clinical findings diagnoses problem lists and care plans legibly and in a timely manner per legal and protocol requirements.
  • Update or adjust care plans based on receipt of additional information or patient progress.
  • Explain plan of care and findings to patients; document patient education provided.
  • Assist leadership in program planning and evaluation; contribute to development of nurse practitioner protocols.
  • Provide leadership training and continuing education to clinical nursing staff as needed.
  • Promote improved health in the community; assist with health fairs and screening programs as needed.
  • Communicate effectively with physicians nursing staff patients families and interdisciplinary team members.
  • Complete other tasks as assigned by the supervisor.

Qualifications & Requirements

  • Education: Licensure as a Registered Nurse and Certified Nurse Practitioner (CNP) required. Masters degree in Nursing preferred.
  • Certifications: Current RN license and CNP license in applicable state required. CPR/BLS certification required.
  • Experience: Prior public health or community health experience preferred. Significant clinical experience in prenatal STI family planning pediatrics and/or family practice preferred.
  • Skills: Demonstrated knowledge of nursing practice physical examination techniques disease states diagnostic testing and pharmacology. Ability to perform and interpret physical assessments formulate differential diagnoses and develop patient management plans. Excellent clinical judgment with ability to know limits of practice and when to seek consultation. Demonstrated ability to work without direct supervision. Strong communication and interpersonal skills.

Working Conditions

  • Work in a clinical setting with potential exposure to communicable diseases bloodborne pathogens and other biohazards.
  • Communicate and interact with physicians nursing staff patients families and the public on a daily basis.
  • Prepare for the possibility of unexpected medical or crisis situations to occur.
  • May be required to adjust work hours location or duties in the event of a public health emergency.

Physical Requirements:
Must be able to lift and carry up to 15 pounds. Position requires standing less than 33% of time on duty walking less than 33% of time on duty and sitting between 33% and 66% of time on duty. Use of hands to finger handle or feel over 66% of time on duty. Reaching with hands or arms less than 33% of time. Full ability to talk and hear. Correctable vision required.
