Physical Therapist (Contract)

Position Summary

The Physical Therapist is responsible for evaluating planning and implementing individualized therapy programs to restore function improve mobility and enhance the overall quality of life for patients. This role requires strong clinical expertise patient-centered care and collaboration with interdisciplinary teams.

Work Schedule

Shift: Monday to Friday 08:00 AM 05:00 PM
Hours: 40 hours per week
Additional: Some holidays and one weekend per month may be required

Key Responsibilities

Perform comprehensive initial and ongoing patient assessments to determine functional limitations and therapy needs.
Develop implement and regularly evaluate individualized plans of care in collaboration with patients and their families.
Deliver therapeutic interventions using established physical therapy techniques tailored to patient conditions.
Monitor patient progress and modify treatment plans as necessary to achieve optimal outcomes.
Supervise and guide physical therapy assistants students and support staff.
Maintain accurate and timely documentation in accordance with regulatory and organizational standards.
Ensure a safe clean and supportive therapy environment.

Qualifications & Requirements

Minimum of 2 years of clinical experience as a Physical Therapist.
Experience in a Skilled Nursing Facility (SNF) setting preferred.
Strong clinical reasoning communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
