What Youll Do

• Deliver high-quality physical therapy care in patients homes

• Develop implement and manage individualized care plans

• Collaborate with a multidisciplinary care team

• Maintain timely and accurate documentation

• Travel locally to patient residences (20% of your day)

What Were Looking For

• Bachelors or Masters degree in Physical Therapy (required)

• Active Physical Therapist license in the state of practice

• Minimum of 1 year of healthcare experience

• Valid drivers license and reliable transportation

• Strong time management and self-scheduling skills

• Comfortable using tablets smartphones and EMR systems

• Commitment to meeting documentation deadlines

• A compassionate patient-centered approach to care
