Position Summary The Licensed Practical Nurse (LPN) provides direct patient care and supports the interdisciplinary healthcare team in delivering high-quality patient-centered services. This role requires strong clinical skills adaptability in a dynamic care environment and a commitment to maintaining safety compliance and excellence in patient outcomes.
Work Schedule - Shift: Night Shift 07:00 PM to 07:00 AM
- Schedule: 36 hours per week (full-time; flexible scheduling available)
- Duration: 6 8 week contract
Key Responsibilities - Assist in assessing and evaluating patients physical psychosocial environmental and educational needs documenting responses to care interventions.
- Support the development and implementation of individualized care plans including discharge planning under the guidance of a Registered Nurse.
- Provide direct patient care including medication administration and assistance with daily care activities.
- Monitor document and communicate changes in patient condition to the Registered Nurse and multidisciplinary team.
- Utilize clinical judgment and critical thinking to support patient care decisions within scope of practice.
- Educate patients and families based on established care plans and reinforce teaching provided by the care team.
- Accurately document patient information in electronic medical records (including Epic systems).
- Delegate and accept delegated tasks appropriately in accordance with scope of practice.
- Participate in departmental meetings in-services and ongoing educational opportunities.
- Support process improvement initiatives to enhance patient satisfaction clinical outcomes and operational efficiency.
- Maintain confidentiality and comply with all HIPAA safety and organizational policies.
- Demonstrate flexibility to support staffing needs across units as required.
- Contribute to a positive work environment by adapting to change and maintaining professionalism under pressure.
- Perform additional duties as assigned to support departmental and organizational goals.
Qualifications & Requirements - Active Licensed Practical Nurse (LPN) license.
- Current BLS (Basic Life Support) certification required.
- Experience with telemetry monitoring and Epic EMR systems preferred.
- Fingerprinting clearance required (as applicable).
- Strong communication organizational and interpersonal skills.
- Ability to work effectively in a fast-paced team-oriented environment.
