Summary:

The Social Worker is a key member of the Neighborhood Health Clinic (NHC) team with shared accountability for success of the function. The Social Worker is a primary resource to patients and their families and multidisciplinary care teams. The Social Worker administers pediatric and family-focused psychosocial assessments crisis intervention and case management services. The Social Worker supports care coordination family-centered behavioral health services; and enhances patient care through social work expertise.

Experience

• 3 years of relevant experience providing behavioral support conducting psychosocial assessments coordinating care plans and delivering services to children and families in healthcare school or social service settings.

• Experience may include pre- or post-licensure roles in social service agencies schools hospitals shelters family service and similar organizations.

• Demonstrated experience providing social work services to children and families in healthcare behavioral health education or community-based settings.

Education

• Masters degree in Social Work from an accredited college or university.

Licensure Registration and/or Certification

• Licensed Master Social Worker (LMSW) valid in the state of Texas.

OR

• Licensed Clinical Social Work (LCSW) valid in the state of Texas.
