Job Summary
Job Title: Licensed Practical Nurse (LPN) Long-Term Care
Location: Prairie du Chien WI
Shifts: Evenings (2:00 PM 10:00 PM)
Schedule: 8-hour shifts
Guaranteed Hours: 40 hours/week
Job Summary:
Navitas Healthcare LLC is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide evening care in a long-term care setting. The LPN will ensure patient treatments are delivered efficiently medications are administered safely and patient needs are met throughout the shift.
Key Responsibilities:
- Provide patient-centered care during evening shifts
- Administer medications and treatments as prescribed
- Monitor and document patient health status accurately
- Maintain up-to-date medical records and documentation
- Assist with daily patient care and nursing duties as needed
Qualifications:
- Active Wisconsin LPN license
- Strong communication and organizational skills
- Previous experience in long-term care preferred
