Position Summary
The RN Case Manager is responsible for coordinating patient care discharge planning and utilization review to ensure high-quality cost-effective treatment. This role collaborates closely with physicians patients families and insurance providers to support optimal patient outcomes.
Key Responsibilities
Coordinate patient care and discharge planning
Conduct patient assessments and reassessments
Perform utilization review using InterQual or similar criteria
Collaborate with physicians and interdisciplinary teams
Communicate with insurance providers for authorization and reimbursement
Identify high-risk patients and connect them with appropriate resources
Advocate for patients and support informed care decisions
Facilitate safe transitions to home care or other facilities
Participate in daily rounds and care planning
Qualifications
Active Registered Nurse (RN) license in Massachusetts (required)
Minimum 3 years of acute care experience
Experience in case management discharge planning and utilization review
Strong knowledge of InterQual or similar systems
Bachelor of Science in Nursing (BSN) preferred
Case Management certification preferred
Additional Requirements
Excellent communication and organizational skills
Ability to work independently in a fast-paced environment
Strong multitasking and prioritization abilities
