We reach more than 73000 individuals and families including almost 13000 serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are in living in poverty reflecting the challenges faced by nearly 1.7 million New Yorkers today.

Prospect Plaza is an affordable development in Ocean Hill-Brownsville which provides a total of 394 housing units in three buildings comprisedof one- two- and three-bedroom units. Prospect Plaza Site 3 hasa set-aside to lease to 27 families formerlyresidinginNew YorkCity homeless shelters under the Our Space initiative.
CAMBA Inc. provides case management services via an experienced Program Coordinator. The Program Coordinator assesses the needs of each family member of the 27 households develops a mutually agreed upon referral and service plan with each resident to help ensure stability and to maximize each residents ability to live independently andmonitorsthe residents progress.

Position: Part-Time Case Manager

Reports To: Senior Program Manager

WhatDoesa/the InsertPositionTitleDo:

  • Maintain professional relationships with clients and client confidentiality.
  • Practice Universal Precautions/Standard Protocol & Procedures.
  • Comply withallFederal State City and CAMBA security and privacypolicesintended to protect the security and privacy of individually identifiable health information.
  • Review all documentationestablishingtenants eligibility for program and make file copies.
  • Create andmaintaintenant files.
  • Conduct initial intake and assessment of tenants and tenants families needs and periodic re-assessments.
  • In collaboration with tenants prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals.
  • Assisttenants inattainingtheir goals byidentifyingandlocatingcommunity resources for tenants and by making referrals toappropriate servicesboth within and outside CAMBA.
  • Work with tenants to break through barriers to tenant goals and toassisttenants inadvocating forthemselves and in moving toward self-sufficiency.
  • Recommend and implement strategies to persuade tenants toparticipatemore fully in this process.
  • Monitor tenants progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits and document via progress notes.
  • Participate in case conferences.
  • Follow-upwith tenantsand withreferral organizationsregardingtenant contact and progress with referral organization.
  • Provide all required information for weekly/monthly/quarterly/annual reports.
  • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months;or(b) have not demonstrated a willingness to participate in the process (lost-to-service);or(c) have become ineligible for services (e.g. moved out of building).
  • Act as tenant liaison/tenant advocate with outside organizationsregardingsuch matters as education healthcare housing legal issues etc.
  • Schedule appointments fortenantwith referral organizations.
  • Escort tenants to appointments (educational medical social service etc.)
  • Assisttenants in completing applications for benefits and entitlements and may process applications on tenants behalf.
  • Follow-upwith tenants fora period of timeafter successful completion of their primary goals toassuretenant stability.
  • Evaluateactualliving conditions of tenants through home visits.
  • May prepare marketing materials for the program.
  • May prescreen tenants over the telephone for eligibilityand mayschedule intake appointments.
  • Input tenant data and tenant progress information into automated database.

What are theQualifications for the Position:

EducationLicensesand/or Certifications Required:

  • Bachelors degree (e.g. B.A. B.S.W.)

Minimum Experiences and Skills Required:

  • Experience working with the Seriously and Persistently Mentally Ill (SPMI) population and/or equivalent experience.

Pre and/or Post Employment Requirements:

  • Must be fingerprinted and obtain Criminal History Check (CHC) with Justice Centerfor the Protection of Vulnerable People with Special Needs Ability tomaintainfingerprint clearance throughout the durationoemployment.
  • As a Mandated Reporter you mustimmediatelyreport suspected incidents/cases abuse and neglect to the Vulnerable Persons Central Register (VPCR) Hotline at 1- and complete a CAMBA IncidentReport
  • Flexible hours may be needed occasionally to provide shift coverage until 8 pm.

Compensation: $26.25 hourly
When salary ranges are listed the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and includes but is not limited to experience education budget and/or program size internal equity skills and other factors that may be required for the position and organization.

Status: Part-Time
