Job Summary
JOB SUMMARY:
The Community Support Services Case Manager/Administrative position has dual responsibilities to deliver both behavioral health services directly to residents residing in the community and provide administrative support tasks which provide direct support to the County Directors and the CSS teams. The CSS Administrator will develop systems processes and procedures to improve the operating quality and efficiency of the CSS program. Duties are performed to ensure compliance with regulatory agencies.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Provide training and support to residents in areas such as: skills development medication management work readiness activities and illness management and recovery.
- Provide crisis assessment and intervention.
- Provide termination planning and provide medication and psychosocial education to clients.
- Develops and maintains positive collaboration with other provider agencies serving clients. Maintain knowledge and familiarity with referral sources within the community. Provide consultation when needed.
- Develops and maintains quality accurate clinical documentation and charts for all clients served.
- Maintains agency-required annual trainings including but not limited to client rights and grievances safety training infection control and others as required by OHMAS.
- Remains free from citations by other providers or by consumers for unethical practice and meet standard for own profession.
- Adheres to all organizational and departmental policies including compliance with all behavioral and ethical expectations.
- Participates in supervision as directed by agency policy.
- Any other tasks/responsibilities as directed by Director or assigned designee.
- On-call responsibilities as assigned by the Director.
- Performs other duties as deemed appropriate by the Supervisor/Director.
- Complete and maintain all documentation being sent/received from the IME.
- Maintain and create clinical documentation in EHR system; including completion of forms and scanning of documents.
- File and archive documents as needed.
- Reviews charts for 100% compliance with record standards at least once each quarter or more often as directed.
- Compile Productivity reports generated by County Directorsensuring the implementation of a comprehensive tracking system.
- Track and monitor all residents IRP units of service and coordinate information with CSS team and Director.
- Organize track and maintain IRP and CRNA tracker for the Northern Region.
- Partners with Directors in the review and implementation of policies and procedures.
- Partners with Directors to support internal and external auditing including review preparedness active audits and action plans.
- Prepares and compiles reports as requested and needed by DMHAS and including but not limited to Annex As and QCMRs as generated by County directors.
- Ensure all documentation of Annex As and QCMRs are distributed to DMHAS County Administrators and DMHAS and corporate finance.
- Responsible for providing information and registration of staff for mandatory Rutgers training.
QUALIFICATIONS:
- A Bachelors Degree from an accredited college or university in social work rehabilitative counseling psychology counseling or other related behavioral health or counseling program discipline.
- Aminimum of 2 years working in a community based behavioral health setting.
- Valid driverslicense in the state of residence plus three-yeardriving history and a driving record in good standing.
- $22.76 per hour.
