Job Summary

Are you a compassionate resourceful professional whos passionate about supporting adults as they navigate care and services PCHCs Care Management team is seeking a Social Worker/Care Manager (MHRT/C or eligible) to join our mission-driven interdisciplinary care this role youll build trusted relationships with adult patients coordinate across providers and connect individuals to essential community resources that support stability and well-being. This role is best suited for someone who is patient flexible and energized by navigating complex systems and barriers to care. This position may work in a Designated Healthcare Facility which is subject to the State of Maines immunization requirements for healthcare workers. If hired you will be required to show a Certificate of Immunization or Proof of Immunity against: 1.) Rubeola (measles); Mumps; Rubella (German measles); Varicella; Hepatitis B; and Influenza.

Whats it like to work at PCHC Find out: Full-Time Salaried Monday-Friday 8:00 AM to 4:30 PM (Schedule may have some flexibility depending on final candidates preferences)
NOTE: This position splits time between Helen Hunt Health Center in Old Town and Brewer Medical Center.

Highlights of the position:

  • Provides high-quality care and service to our patients by demonstrating empathy kindness effective communication and advocacy.
  • Serves on an interdisciplinary team to identify and support patients in need of Care Management services performing assessments of clinical and social status and determining medical behavior and social needs. Typical activities include:
    • Collaborates with providers staff and team members on a daily basis.
    • Educates providers regarding the patients needs.
    • Participates in collaboration of patient care by meeting with patients in-person by phone or by Telehealth.
  • Coordinates and communicates with community support and social service systems for the benefit of patients and families maintaining confidentiality of patient records in accordance with HIPAA guidelines and PCHC policies.
  • Establishes and maintains collegial relationships with community providers/contacts to expedite referrals to community resources. Liaisons and advocates with community providers to promote and support patient needs.
  • Engages and supports patients to ensure they feel heard respected and involved in their care.

Education and Experience:

  • MHRT-C required (or eligibility for MHRT-C) for Adult Care Managers
  • Minimum of three years direct experience in social work in a health care setting required.
  • At least one year of case management experience required.
  • Having a valid drivers license with an acceptable driving record and/or an appropriate length of driving experience for insurance purposes is required. Any concerns raised by the results of a driving record check will be subject to an individualized review to determine whether the infraction is relevant to the individuals ability to perform the responsibilities of the position in accordance with PCHCs Automobile Safety and Background Check policy.

  • Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American heart Association American Red Cross or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
