Nemours Childrens Health is seeking an Audiology Patient Navigator (Part-Time) to join our team in Wilmington Delaware.

This is a 3-year grant funded position.

The patient navigator will serve as a key liaison between families medical teams and early intervention programs focusing on children newly diagnosed with hearing loss. This role is highly collaborative and involves:

  • Guiding Families:Supporting families from the point of referral after newborn hearing screening educating them on the importance oftimelyfollow-up and intervention (the 1:3:6 benchmarks) and helping them navigate healthcare appointments and early intervention systems.
  • Coordinating Care:Scheduling appointments sending orders and documenting family interactions in the electronic medical record system.
  • Data Management:Collecting and analyzing patient data using the Nemours outcomes database tracking program.
  • Interdisciplinary Collaboration:Working closely with the Outcomes and Outreach Coordinator Audiology department Department of Public Health Birth to Three program and attending EHDI board meetings.
  • Communication:Maintainingsecure andtimelycommunication with families and external partners to ensure follow-up screenings and diagnostic evaluations are completed.
  • Advocacy & Support:Serving as a resource for families providing information about hearing loss early intervention and communitysupport andparticipatingininitialIFSP meetings as needed.

Essential Functions:

  • Additional miscellaneous duties and responsibilities as may be assigned from time to time by employees supervisor.
  • Interact and Educate on Support Systems to Maximize Patient Support:Staying current with federal and state early childhood intervention and special education law with regards to hearing and speech.
  • Manage and Track Referrals/Outcomes:Provide analysis and trending information to senior leadership team members and others as requested regarding loss to follow up data screening/diagnosis/treatment of hearing loss data and early intervention enrollment.
  • Coach Patient/Family Learning:Provide counseling and education and resources to ensure that children who have failed a newborn hearing screening are getting timely follow up care. Active involvement in community educational events regarding newborn hearing screenings and importance of early identification of hearing loss.
  • Manage Continuous Communication:Manage records and reports including EPIC notes documenting communications with families. Working with ENT advanced practice providers physicians and others involved with care to ensure smooth and seamless patient flow for communication related programs.

Job Requirements:

  • Bachelors degree is required. Bachelors degree in health-related field preferred.
  • Minimum of one (1) to three (3) years experience required.
  • Working with children and families in healthcare experience with early intervention and hearing loss and 1 year of scheduling experience is preferred.
