JOB TITLE:
Clinical Assistant Professor - Pharmacist Geriatrics Substance Use Disorders & Community Pharmacy MTM
JOB PURPOSE:
The Pharmacist provides patient-centered medication management with primary responsibilities in geriatric pharmacotherapy substance use disorders (SUD) and community pharmacy medication therapy management (MTM). This position serves as a core preceptor for an ASHP-accredited pharmacy residency program contributing to resident education evaluation and professional development. The pharmacist functions as an integral member of the interprofessional healthcare and education team.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide comprehensive medication management for older adults including deprescribing and polypharmacy reduction using evidence-based tools.
- Manage and monitor pharmacotherapy for substance use disorders including medications for opioid use disorder (MOUD) and harm-reduction strategies.
- Deliver community pharmacy MTM services including comprehensive and targeted medication reviews documentation and provider communication.
- Identify and resolve medication-related problems related to safety effectiveness adherence and cost.
- Serve as a pharmacy residency preceptor in accordance with ASHP Accreditation Standards including resident mentoring teaching and evaluation.
- Participate in residency recruitment orientation assessment and continuous quality improvement.
- Contribute to clinical service development quality improvement and policy or guideline initiatives.
- Collaborate with physicians behavioral health providers and other healthcare professionals to support patient-centered care.
SUPPLEMENTAL FUNCTIONS:
The Pharmacist will also provide coverage of pharmacy staffing and other ambulatory care service needs.
COMPETENCIES:
- Advanced clinical expertise in geriatric pharmacotherapy including polypharmacy management and deprescribing
- Clinical competence in substance use disorder treatment including medications for opioid use disorder (MOUD) and harm-reduction strategies
- Proficiency in community pharmacy medication therapy management (MTM) including comprehensive and targeted medication reviews and documentation
- Ability to identify resolve and communicate medication-related problems affecting safety effectiveness adherence and cost
- Strong patient-centered communication and education skills particularly with older adults and vulnerable populations
- Effective interprofessional collaboration within a family medicine residency and integrated care team
- Competence in pharmacy residency precepting and teaching in alignment with ASHP standards
- Commitment to quality improvement professional ethics and continuous learning
MINIMUM QUALIFICATIONS:
Doctor of Pharmacy (PharmD) from an ACPE-accredited college or school of pharmacy.
Active pharmacist license in Wyoming or eligibility for licensure.
Completion of a PGY1 pharmacy residency.
Eligibility to serve as a residency preceptor per ASHP standards.
Strong communication documentation and collaboration skills
Valid drivers license with a motor vehicle record (MVR) compliant with the Driving for University Business SAP.
DESIRED QUALIFICATIONS:
PGY2 residency in Ambulatory Care Geriatrics or Psychiatry.
Board certification (e.g. BCGP BCACP BCPS) or eligibility.
Experience in geriatrics substance use disorders and/or community pharmacy MTM.
Prior experience precepting pharmacy students or residents.
