Position: Full Time Case Management Coordinator

PURPOSE
The Case Management and Swing Bed Coordinator is responsible for assisting the Director with the management of day-to-day operation of the Case Management Department. The Coordinator creates a common vision to facilitate continuous quality improvement in the delivery of patient care in support of the organizations mission and values. The Coordinator duties may include managing a daily assignment in addition to a few managerial responsibilities. The Coordinator will assist the Director in providing leadership in clinical case management services to maximize the patient-centered outcomes across the patient-care continuum from pre-admission through post-discharge. Case management services include monitoring patient care to ensure progress-desired outcome addressing patient and family needs resolving obstacles to effective care obtaining authorizations for services from third party payors and planning for patient discharges.
QUALIFICATIONS
- An Associate or Bachelors degree in nursing is required.
- If RN current Ohio license required.
- A Certified Case Management designation is desired.
- Three to five years experience in a healthcare setting.
- Interpretive skills frequent interactive and consultative associations are inherent to this position.
- The ability to retrieve communicate or otherwise present information in a written auditory or visual fashion is essential.
