Job Responsibilities:

  • Participate in planning related to resident family and community interests. Participate in discharge planning development and implementation of social care plans and resident assessments. Perform administrative duties such as completing medical forms reports evaluations studies charting arranging transportation etc. as necessary coordinating activities with other departments as necessary.

  • Promotes optimum transition of resident for appropriate and safe placement for discharge. Provides ongoing communication with resident and responsible persons of progress/status during stay.

  • Provide information to the resident/family on Medicare/Medicaid as well as other financial assistance programs available. Provide consultation to staff community agencies efforts to solve resident concerns through the development of social service programs.

Minimum Qualifications:

  • Must possess a bachelors or masters degree in social work or a bachelors or masters degree in a human services field (i.e. sociology special education rehabilitation counseling and psychology.)

  • One year of supervised social work experience in a healthcare setting working directly with individuals.

  • Experience in long-term care working with the senior population and dementia counseling.

  • Must be licensed as a social worker in Michigan.

  • Must lead by example and possess superior customer service skills and professionalism.

  • Flexibility to adapt to ongoing change and work in a fast-paced customer driven environment.
