Job Summary
Job Title: Social Worker - Comp Psych Emerg Prog Cpep - Mount Sinai West - Full Time - Day (Tuesday-Saturday: 10am-6pm)
- Assesses and evaluates patients/families social psychosocial and environmental situations to determine patient needs.
- Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned.
Responsibilities
- Develop options and treatment plans in a timely manner.
- Provide direct service to patients and families selecting treatment modality casework technique compatible with assessment and identified goals.
- Ability to identify select access and make referrals to appropriate community resources.
- Monitor coordinate manage activities for high risk psychosocial patients to ensure implementation of multiple services.
- Advocate for patients and families by interpreting their needs to and eliciting cooperation from other disciplines and community agencies.
- Identify and assist team in managing obstacles to treatment and/or discharge plans.
- Provide psycho-education for patients and families.
- Knowledge of child protection elder abuse domestic violence policies and protocols.
- Participate interdisciplinary rounds case conferences staff meetings.
- Document in the medical record in accordance with standards set by Department of Social Work and/or regulatory agencies.
- May supervise social work interns.
Qualifications
- Masters in Social Work from an accredited school of social work. Knowledge of social work services theories techniques community resources and agencies.
- Related experience in the social service and/or health care field preferably one year in a medical setting which can include related field placement.
- New York State Department of Education: Licensed Masters of Social Work (LMSW).
