We are looking for a dedicated and proactive Field Supervisor to join our growing team. This is a varied and rewarding role that combines both office-based responsibilities and hands-on fieldwork giving you the opportunity to make a real impact on the quality of care delivered to service users.
Working 5 days per week (including 1 weekend in 2) you will play a key role in supporting care delivery maintaining high standards and building strong relationships with staff service users and external healthcare professionals.
Your responsibilities will include:
- Building positive and supportive relationships with staff service users and their families
- Working in partnership with District Nurses GPs and other healthcare professionals
- Attending new service user assessments and setting up care packages
- Creating and updating care plans and risk assessments
- Ensuring accurate record-keeping and system updates (Empower/Care Planner)
- Managing and responding to calls queries and service enquiries
- Supporting rota management and ensuring all care visits are covered
- Completing quality assurance checks (home and phone-based)
- Conducting audits medication checks and ensuring compliance standards are met
- Carrying out staff supervisions appraisals and competency assessments
- Supporting and mentoring care staff in the field
- Participating in on-call duties (1 week in 2 including weekends)
- Attending weekly KPI meetings and contributing to continuous improvement
This is a dynamic role where no two days are the same offering the opportunity to take ownership work independently and contribute to high-quality care delivery.
Requirements
- Previous experience in a care or supervisory role
- Strong ability to work on your own initiative
- Excellent communication and relationship-building skills
- Good reporting and organisational skills
- Ability to manage multiple tasks and respond to changing priorities
- Confident using care systems and maintaining accurate records
- Understanding of care planning risk assessments and medication processes
- Willingness to participate in on-call duties and flexible working when required
- Must be able to drive
