Our growing Liver Clinic is seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner) to join our multidisciplinary hepatology team. This role provides comprehensive care for patients with a wide range of liver conditions including cirrhosis hepatitis fatty liver disease autoimmune liver disorders pre- and post-transplant care and other complex hepatology needs. The APP will work collaboratively with hepatologists nursing and support staff to deliver high-quality patient-centered care. With the goal after training to see 12-15 patients per day pending complexity.

• Monday-Friday 8-4:30
• Conduct new and follow-up patient visits including assessments diagnosis treatment planning and ongoing management of liver-related conditions.
• Coordinate care for high-acuity and chronically ill patients ensuring seamless communication across the care team.
• Order and interpret diagnostic tests labs imaging and specialty evaluations.
• Manage medication adjustments including hepatology-specific therapies and monitoring for side effects.
• Provide patient and family education related to liver disease treatment plans lifestyle interventions and long-term care needs.
• Participate in care pathways for cirrhosis management transplant evaluations and post-transplant follow-up as applicable.
• Collaborate with inpatient teams to support transitions of care and reduce readmissions.
• Document all patient interactions in the electronic medical record accurately and in a timely manner.
• Contribute to quality improvement initiatives care standardization and process development within the clinic.
• Outreach clinic at least once monthly and could involve more pending volumes.

• Licensed Nurse Practitioner (NP) in the state of Iowa
• Minimum of 2 years of experience in GI/hepatology internal medicine or a related specialty preferred.
• Demonstrated ability to manage complex patients in an outpatient specialty setting.
• Strong communication critical-thinking and patient education skills.
• Ability to work effectively in a multidisciplinary team.
• EMR proficiency (Epic preferred).
