Job Summary

Compensation Range: $18/hr - $19/hr commensurate with experience

Benefits:

Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. Overview:

The Part-Time Care Navigator supports Housing Specialists in providing comprehensive client-centered services to individuals living with HIV. This role focuses on helping clients access housing resources supportive services and healthcare while promoting housing stability and improved health outcomes. The Care Navigator works closely with clients to identify barriers coordinate services and connect them with community resources. The Part-time Care Navigator works with clients in the Ryan White HPC Housing Placement Program and serves as a role model and advocate in the areas of entitlement self-advocacy empowerment navigating systems health and behavioral health and housing.

Responsibilities:

  • Provide concrete service delivery to clients as per program needs (i.e. benefits entitlements accompaniments to medical behavioral health and housing appointments) and connect them to community-based resources.
  • Secure linkage to medical care and essential support services for individuals who test HEP C positive.
  • Conduct outreach and engagement field work for clients who need further program engagement and work with clients to identify barriers to engagement.
  • Conduct home/field visits and maintain client engagement in accordance with program standards.
  • Coordinate client services with internal and external service providers as directed by team leadership (field based or office based).
  • Document client encounters in electronic health records.
  • Perform other duties as may be assigned.

Requirements:

  • High School Diploma or GED required
  • Good oral and written communication skills as well as good interpersonal and customer service skills and the ability to effectively work as part of a team.
  • Demonstrated computer proficiency particularly with Microsoft Office and electronic health record systems.
  • Demonstrated knowledge of HIV/AIDS chronic medical conditions mental illness substance use and homelessness as well as a basic understanding of public benefits and entitlements.
