JOB SUMMARY
Promotes advocates and ensures the adequacy and safety of care received and the quality of life of older adult residents of assisted living and long-term care facilities in Salt Lake County in accordance with applicable Federal and State laws regulations and policies.
MINIMUM QUALIFICATIONS
Bachelors degree from an accredited college or university in a closely related field; OR an equivalent combination of related education and experience.
Must be able to obtain certification as a certified Ombudsman within six (6) months of hire.
Due to the nature of this position the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.
ESSENTIAL FUNCTIONS
- Encourages elderly residents and family members to advocate positive change in the facility.
- Provides information and referrals to individuals regarding assisted living and long-term care facilities.
- Provides information to the public residents community organizations and other agencies about the needs/rights of elderly residents.
- Investigates reports of alleged inadequacy of care and violations of rights abuse neglect or exploitation of residents.
- Coordinates and cooperates with other agencies to protect elderly residents.
- Conducts consultations and works to resolve complaints through an action plan.
- Provides advocacy to correct problems individually and on a systemic level. Ensures that interventions focus on the quality of care and life.
- Assesses and evaluates residents needs to make appropriate recommendations and referrals.
- Coordinates services with Medicaid Fraud Adult Protective Services State Licensing agency and certifies assisted living and long-term care facilities and other appropriate agencies.
- Creates and maintains case records.
- Ensures the timely entering maintenance and submission of activities and case work into State Ombudsman software programs.
- Provides specialized technical assistance consultation training and resources to providers and community organizations.

KNOWLEDGE SKILLS AND ABILITIES (KSA)
Knowledge of:
- Ombudsman policies and procedures
- State of Utah rules and regulations of assisted living and long-term care facilities
- Investigative techniques
- Medicare Medicaid and other insurance information and community resources
- Federal law regarding resident rights
- Care planning for long-term care residents
- Basic knowledge of medical conditions common to geriatric residents
- Microsoft Office
Skills and Abilities to:
- Interpret and communicate complex rules regulations and policies and procedures
- Communicate effectively both verbally and in writing
- Relate to individuals with diverse backgrounds
- Maintain and organize case files and reporting elements
- Learn and access the Ombudsmen manager database and forms
- Act independently and assertively
- Develop working relationships with facility staff government and community organizations
Additional Information
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS
Must be able to access transportation to perform assigned work tasks in various community locations within a reasonable time.
Required to travel throughout Salt Lake County on a daily basis.
IMPORTANT INFORMATION REGARDING THIS POSITION
Candidates cannot serve as an Ombudsman and work in a licensed assisted living or licensed nursing care facility at the same time due to state and federal policy.
